总 则为加强建筑筑装饰工工程施工工现场安安全管理理,提高高工程施施工现场场安全管管理水平平,防止止和减少少生产安安全事故故,保障障现场相相关人员员生命和和财产安安全,特特制定本本LBBY安全全生产管管理手册册(以以下简称称安全全管理手册册)。本手册依据据中华华人民共共和国安安全生产产法、中中华人民民共和国国建筑法法、建建设工程程安全生生产管理理条例等等国家现现行的有有关工程程建设的的法律、法法规、规规范、强强制性标标准和有有规定编编制。本手册适用用于北京京丽贝亚亚建筑装装饰工程程有限公公司在装装饰装修修项目施施工过程程中,对对施工现现场安全全生产的的管理。凡凡参与工工程建设设施工的的项目均应应执行本本手册。北京丽贝亚亚建设装饰饰工程有有限公司司施工现现场安全全生产管管理在执行本本手册的的同时,还还应符合合国家现现行有关关法律、法法规、规规范、标标准和项项目所在在地政府府部门的的有关规规定。二、 安全生产管管理体系系1.公司领领导决策策层依法法对本公公司的安安全生产产工作全全面负责责,公司总总经理是是本公司司安全生生产的主主要责任任人,项项目经理理是工程程项目部部安全生生产的第第一责任任人;分分管安全全生产的的副总经经理具体体负责指指导和落落实本公公司的安安全生产产管理工工作,公公司技术术中心监监察部负负责公司司直营项项目安全全生产监监督管理理。2.各项目目经理部部必须设设立安全全生产领领导组织织机构和配配备专职职安全生生产管理理人员,各各施工班班组必须须配备专专职或兼兼职安全全生产管管理人员员,项目目部专职职安全员员和施工工班组专专职或兼兼职安全全生产管管理人员员由项目目经理验验证核实实。3.项目部部专职安全全员和施施工班组组专、兼职职安全员员,业务务管理归归口公司司技术中中心项目目监察部,日常管管理归口口项目经经理部。4.项目部部安全管管理体系系结构图图 10003300人人必须有有专职安安全员11名;c 30005500人人必须有有专职安安全员22名,纳纳入总包包安全部部统一进进行业务务指导和和管理。三、 安全生产职职责劳务班组长劳务安全员1.项目经经理安全全生产职职责1.1项目目经理是是工程项项目安全全生产的的第一责责任人,对对工程项项目的安安全生产产负直接接领导责责任;项项目经理理必须把把安全生生产工作作列入重重要议事事日程,每每月至少少召开二二次安全全生产会会议,分分析研究究当前安安全生产产情况和和特点,提提出防范范整改措措施,并并有会议议记录。1.2负责责建立和和完善工工程项目目安全生生产组织织体系,成成立安全全生产与与文明施施工领导导小组(由由技术、工工程、安安全负责责人组成成),并并领导其其有效运运行。1.3认真真执行安安全生产产管理规规章制度度,按要要求确定定本项目目的安全全生产和和文明施施工管理理目标和和安全生生产工作作计划;在编制制、审批批、布置置本项目目施工生生产工作作的同时时,编制制、审批批、布置置本项目目的安全全生产工工作。1.4按照照“管生生产必须须管安全全”的原原则,组组织编制制施工安安全生产产措施计计划,确确保工程程项目为为安全生生产所需需经费的的合理投投入,在在下达施施工生产产计划的的同时,必必须下达达安全生生产技术术措施计计划,以以确保工工程施工工安全生生产。1.5组织织并参加加定期的的安全生生产检查查,做到到定人、定定时、定定措施落落实隐患患整改,保保证生产产设备、安安全装置置、消防防设施、防防护器材材和急救救器具等等处于完完好状态态。1.6遇事事故发生生,及时时报告安安全事故故,负责责事故现现场保护护与抢救救工作,配配合有关关部门进进行事故故调查和和处理,按按照“四四不放过过”的事事故处理理原则做做好相关关工作,吸吸取经验验教训。1.7对省省、地、市市(县)各各项安全全生产与与文明施施工检查查,要力力争创先先,并确确保实现现既定的的安全生生产管理理目标。2.项目技技术负责责人安全全生产职职责2.1在项项目经理理的领导导下,贯贯彻执行行上级有有关施工工现场各各项管理理规定,对对项目施施工安全全技术负负全面领领导责任任,把安安全技术术工作列列为重要要工作内内容。2.2协助助项目经经理制定定文明安安全施工工计划并并组织实实施,对对全体员员工进行行文明安安全生产产教育和和组织安安全生产产大检查查,确保保安全文文明生产产顺利进进行。2.3在编编制施工工组织设设计(方方案)时时,要贯贯彻“安安全第一一、预防防为主”的的方针;确定施施工方法法,运用用机械设设备等每每个环节节,都要要有安全全技术措措施,针针对性强强,成为为科学指指导施工工的技术术依据。2.4对采采取的新新技术、新新工艺、新新设备,负负责制订订相应的的安全技技术措施施。2.5积极极推行安安全目标标管理,对对施工中中存在的的各种安安全难题题,有计计划地组组织力量量,攻克克技术难难关,保保障生产产安全,改改善劳动动条件。2.6 参参加事故故调查,从从技术角角度分析析事故原原因,提提出初步步处置意意见和改改进措施施。3.专职安安全员安全生产职职责3.1认真真贯彻安安全生产产法律、法法规,严严格遵守守公司安安全生产产规章制制度及各各工种安安全技术术操作规规程,坚坚持“安安全第一一、预防防为主、综综合治理理”的方方针,对对项目部部班组作作业人员员开展经经常性安全生生产和文文明施工工的思想想教育活活动(每每周一次次)。3.2推行行工程项项目标准准化管理理,组织织建立 QC 小组,通通过 QQC 小小组活动动把安全全隐患消消除在萌萌芽状态态,杜绝绝重大安安全事故故发生。3.3负责责对班组组进行安安全技术术交底。3.4负责责落实安安全技术术措施,落落实做好好本项目目部的临临时设施施、架体体、设备备、电气气等安全全防护工工作。3.5巡回回检查施施工现场场的安全全生产情情况,及及时发现现和排除除事故隐隐患,不能解解决的立立即报告告,与项项目负责责人一道道采取有有效措施施处理,防防止事故故发生;做好安安全检查查原始记记录。3.6认真真执行安安全纪律律,有权权制止违违章指挥挥和违章章作业,予予以纠正正不按规规定穿戴戴安全防防护用品品,不听听劝阻的的可令其其停止工工作。3.7及时时报告工工伤事故故,保护护好事故故现场,参参加因工工伤亡及及重大未未遂事故故的调查查,认真真分析事事故原因因,提出出处理意意见,制制定落实实防范措措施。3.8经常常保持工工地安全全标语牌牌及各种种禁令标标志的完完好无损损,做好好工地文文明施工工工作。3.9协助助领导组组织应急急抢险队队,做好好防火、防防塌、防防自然灾灾害的抢抢险工作作,保护护生命财财产安全全,减少少灾害损损失。4.班组长长(兼职职安全员员)安全全生产职职责4.1各工工种班组组长兼任任班组安安全员,执执行公司司和项目目部对安安全生产产的规定定和要求求,全面面负责本本班组的的安全生生产,抓抓好本工工种安全全生产和和文明施施工工作作,并有权权督促其其它班组组共同做做好配合合工作。4.2负责责对本班班组职工工进行岗岗位安全全教育;组织进进行安全全生产活活动,坚坚持班前前讲安全全、班中中检查安安全、班班后总结结安全,在在交施工工任务的的同时交交底安全全生产,并并根据作作业环境境和职工工的思想想、体质质、技术术水平合合理分配配生产任任务。4.3负责责本班组组的安全全生产检检查,上上班前,班班组长应应对作业业环境(包包括脚手手架、井井字架、机机械、电电气、照照明、通通道、易易燃场所所等)进进行安全全检查,遇遇有险情情必须及及时处理理。4.4遵纪纪守法,杜杜绝违章章指挥,制制止违章章作业,及及时消除除不安全全因素;发生工工伤事故故立即报报告,并并组织抢抢救,做做好事故故现场保保护工作作,并作作好详细细记录及及时报告告。4.5搞好好生产设设备、消消防器材材、防护护设施的的检查维维护工作作,使其其经常保保持完好好和正常常运行;教育职职工合理理使用劳劳动防护护用品,正正确使用用灭火器器材。四、安全生生产管理理制度1.安全生生产责任任制1.1项目目管理人人员安全全生产责责任制编编制,项项目经理理开工110日内内完成。1.2安全全生产责责任制应应经责任任人签字字确认;1.3项目目管理人人员花名名册建立立,安全全员开工工5日内内完成。1.4项目目部应有有各工种种安全技技术操作作规程,技技术负责责人开工工10日日内完成成。1.5项目目部应按按规定配配备专职职安全员员,项目目经理项项目开工工前前确确认。1.6对实实行经济济承包的的工程项项目,承承包合同同中应有有安全生生产考核核指标,项项目经理理10日日内完成成。1.7项目目部应严严格按照照公司用用于安全全生产资资金保障障的相关关规定进进行落实实;1.8项目目部应制制定以伤伤亡事故故控制、现现场安全全达标、文文明施工工为主要要内容的的安全生生产管理理目标,项项目经理理开工110日内内完成。1.9责任任目标的的考核以以半月一一次的安安全评分分为依据据(见:项目安安全检查查评分表表)2.施工组组织设计计及专项项方案审审批2.1施工工组织设设计、专专项方案案、项目目策划编编制及审审批,技技术负责责人进场场后200日内完完成。3.安全技技术交底底3.1分部部分项工工程安全全技术交交底,安全员员作业开开始前22天。3.2专项项安全技技术交底底,安全员员在作业开开始前22天。3.3安装装(拆除除)安全全技术交交底,安安全员在在作业开开始前22天。4.安全检检查4.1项目目安全检检查制度制制定,项项目经理理开工77日内完完成。4.2安全全隐患整整改通知知下达,安安全员安安全检查查当日完完成。4.3隐患患整改情情况复查查验证,安安全员整整改期限限内完成成。5.安全教教育5.1制定定安全教教育与培培训制度度;项目目经理开开工5日日内完成成。5.2建立立职工安安全教育育培训花花名册,安安全员开开工100日内完完成。5.3健全全企业职职工安全全教育档档案,安安全员开开工155日内完完成。6.班前安安全活动动6.1建立立班前安安全活动动制度,项项目经理理进场55日内完完成。6.2完善善班前安安全活动动记录,由由安全员员每周周周检时检检查。7.特种作作业持证证上岗7.1特种种作业人人员证书书集中管管理,安安全员开开工155日内完完成。7.2 编编制特种种作业人人员花名名册,安安全员开开工100日内完完成。8.伤亡事事故报告告及调查查处理8.1伤亡亡事故报报告处理理,认真真执行企企业职工工伤亡事事故分类类标准GGB64441-86标标准。事事故处理理,坚持持“四不放放过”原则(即即:事故故原因未未查清不不放过;事故责责任者和和群众没没有受到到教育不不放过;没有制制定出防防范措施施不放过过和事故故责任者者没有受受到处理理不放过过)。对于未未按国家家法律法法规、标标准、规规范和企企业安全全管理规规定进行行项目安安全生产产管控,导导致生产产安全责责任事故故的,追追究相关关责任人的的相应责责任。8.2施工工现场发发生任何何事故都都必须按按事故应应急预案案要求,积积极抢救救伤员、保保护现场场,正确确处理,防防止事故故继续扩扩大和蔓蔓延。项项目经理理和项目目专职安安全员应应立即报报告中心心、事业业部总经经理及公公司领导导,以便便采取有有效措施施,减少少经济损损失,同同时吸取取教训,举举一反三三,杜绝绝事故再再次发生生。8.3填写写施工现现场职工工伤亡事事故双周周报表,安安全员每每月155日、330日以以前。8.4伤亡亡事故及及重大未未遂伤亡亡事故记记录,安安全员发发生事故故随时记记录。8.5办理理劳务意意外伤害害保险,项项目经理理在劳务务进场220日内内完成。9.生产安安全事故故分类(LLBY标标准)9.1重大大事故:死亡11人,或或者重伤伤3人,或或者800万以上上直接经经济损失失的事故故;9.2较大大事故:重伤11-2人人,或者者轻伤55人以上上,或者者20-80万万元直接接经济损损失的事事故;9.3一般般事故:轻伤11-4人人,或者者直接经经济损失失2-220万元元的直接接经济损损失事故故。10事故故报告a一般事故故发生时时,当事事人或相相关人应应立即向向项目经经理报告告,项目目经理报报告项目目中心、事事业部、技技术中心心;b较大事故故发生时时,当事事人或相相关人应应立即向向项目经经理报告告,项目目经理报报告项目目中心、事事业部、技技术中心心、工管中中心;c重大事故故发生时时,当事事人或相相关人应应立即向向项目经经理报告告,项目目经理报报告项目目中心、事事业部、技技术中心心、工管管中心,由由技术中中心、工工管中心心上报总总经理。d一般事故故和较大大事故发发生时,当事人或相关人应在2小时按程序完成上报;重大事故发生时,相关人必须按照事故报告程序立即上报;遇特殊情况可越级上报。11.安全全标志11.1编编制施工工现场安安全标志志布置总总平面图图,安全全员开工工20日内内编制,随随施工进进度调整整、更新新。11.2安安全标志志的使用用管理参参照公司司20111版安安全文明明施工标标识牌应应用手册册12.安全全防护、临临时设施施管理12.1落落实安全全防护、临临时设施施管理制制度,项项目经理理开工220日内内完成。12.2安安全防护护用具及及设备设设施机具具准用管管理,安安全员每每周整理理一次。12.3 个体防防护用品品采购必必须符合合国家标标准,使使用发放放建立台台帐。13.设备备设施、机机具验收收及检测测记录13.1临临水、临临电验收收,由项目目部组织织实施,未未经验收收合格不不得投入入使用。13.2落落地式扣扣件钢管管脚手架架搭设和和拆除,严严格执行行JGJJ1300-20011标标准;门门式脚手手架搭设设与拆除除,严格格执行JJGJ1128-20110标准准;高处处作业吊吊篮执行行GB1191555-220033;悬挑挑式脚手手架、物物料提升升机搭设设、安装装和拆除除应执行行相关标标准和规规程,填填写验收收表,技技术负责责人及相相关验收收人当日日完成,未未经验收收合格不不得投入入使用。13.3租租赁机械械、设备备设施安安装应由由专业承承包单位位负责,由由项目部部组织相相关单位位共同验验收,未未经验收收合格不不得投入入使用。14.文明明施工14.1落落实文明明施工管管理制度度。14.2 落实消消防制度度,消防器器材布置置,安全全员开工工30日日内完成成。14.3编编织安全全生产应应急预案案,安全全员开工工20日内内完成。14.4组组织消防防演习(影影像记录录),安安全员开开工300日内完完成。14.5落落实动火火审批制制度,未未经审批批不得动动火作业业。14.6落落实治安安保卫制制度。14.7标标识标牌牌参照公公司20011版版安全全文明施施工标识识牌应用用手册14.8安安全文明明施工标标识牌费费用投入入规定a对于合同同金额在在10000万元元人民币币以下的的施工现现场,安安全文明明施工标标识牌投投入金额额要求550000 元b对于合同同金额110000万30000万万元,施施工现场场安全文文明施工工标识牌牌投入金金额要求求500001150000元c对于合同同金额330000万50000万万元,施施工现场场安全文文明施工工标识牌牌投入金金额要求求150000250000 元d对于合同同金额 50000
