The purpose of this procedure is to instruct all managers, discipline engineers, supervisors, foreman and also the workers in the basic step-by-step procedure conducting a JHA.工作危险性分析（JHA）是为了减小、消除和控制操作和工作执行期间的风险而进行的正式的检查和实施的程序。本程序的目的是为所有的管理人员、专业工程师、主管、班组长以及工人提供进行工作危险性分析（JHA）的基本步骤。The JHA Process is considered the most sophisticated method and most commonly used when existing safety management controls are insufficient to guarantee a safe and efficient job.当现有的安全管理措施不足以保证安全的和有效的工作时，进行工作危险性分析被认为是最完善的方法，也是最通用的方法。2 GENERAL 概述A Job Hazard Analysis (JHA) involves the detailed and systematic review of a given task, resulting in the identification and understanding of:工作危险性分析（JHA）包括对指定任务进行详细的和系统的检查，以便认识和理解下列各点：Ø The sequence and events or activities to be followed.应遵循的作业次序和事项或活动。Ø The specific job hazards associated with each activity.与每项作业有关的具体的工作危险性。Ø The degree or magnitude of risk associated with each activity.与每项作业有关的风险程度或大小。Ø The control measures required to eliminate and control the associated risks.消除和控制有关风险所需的控制措施。Ø A safe work procedure that can be followed by all personnel which includes the precautions needed to be taken whilst performing the work.所有人员应遵循的安全工作程序，包括执行工作期间需要采取的预防措施。2.1 When to perform a JHA 何时进行工作危险性分析Ø A Job Hazard Analysis must be performed for each and every new activity that contractors and sub-contractors intend to perform对于承包商和分包商需要执行的每项新的工作，都必须进行一次工作危险性分析。Ø If there is no existing safe work practice for a particular task or work activity or if existing safe work practices are not sufficient to control the risks, conduct a JHA.如果对于一项特定的任务或作业没有现成的安全工作规程，或者如果现有的安全规程不足以控制风险，那么就要进行一次工作危险性分析。Existing JHA have to be reviewed for jobs that are repeated, however care must be taken to ensure the conditions are the same as before.对于重复的作业，必须审核现有的工作危险性分析，但是必须注意确保作业条件应和上一次作业相同。2.2 How to prepare to conduct a JHA 如何准备进行工作危险性分析List all fundamental risks and related questions列出所有的基本风险以及有关的问题。Ø What is the activity to be performed?要执行的工作是什么？Ø What tools, equipment and materials are required?需要什么样的工具、设备和材料？Ø What qualifications and training are required?需要什么样的资质和培训？Ø Who is best suited to accomplish the activity?谁最适合完成这项工作？Ø What could go wrong?隐患可能会出在哪里？Ø Who could be affected?谁可能会受到影响？Ø How serious could the negative effects be?会有什么样的一连串负面影响？Ø How likely is the negative effect to actively occur?负面影响发生的可能性有多大？Ø What can be done or implemented to prevent incidents or accidents from occurring采取什么措施来防止事件或事故的发生？2.3 Job Hazard Analysis (JHA) Process工作危险性分析（JHA）过程Ø Describe activity to be done.描述将要做的工作Ø List tools, equipment and materials required.列出所需的工具、设备和材料Ø List the major task or steps in sequence.列出主要任务或按顺序列出步骤Ø Identify the hazards and risks with each task or step.识别每项任务或步骤所伴随的危险性和风险Ø Estimate the risk for each task or step.评估每项任务或步骤的风险Ø Define the method of control to eliminate or reduce the risk.详细说明消除或减小风险的控制方法Ø Determine if the control has minimized the risk to an acceptable level.确定控制措施是否已将风险减小到了可接受的程度Ø Re-confirm that each activity / task is in a logical sequence.重新确认每项工作/任务是否符合逻辑顺序Job Hazard Analysis (JHA) should be conducted by the person responsible for the activity along with supervisors and personnel that will be involved in the doing of the work. The completed Job Hazard Analysis (JHA) must be agreed to and signed off by the following;工作危险性分析（JHA）应当由负责完成工作的人和主管以及参与工作的人员一起进行。完成后的工作危险性分析（JHA）必须由下列人员认可并签字：Ø Contractor JHA prepared by and reviewed by.承包商JHA准备人和审核人Ø Project Construction Manager项目施工经理；Ø Construction superintendent工程监理；Ø Construction HSE Manager施工HSE经理。Make sure ALL personnel who will participate in the work are represented in the JHA process, New JHA meeting must occur for personnel changes.
