转化生长因子-1论文:2型糖尿病肾病患者血清TGF-1和PDGF-BB水平与尿白蛋白排泄率的相关性研究【中文摘要】2型糖尿病(T2DM)患者血清转化生长因子1 (TGF-1)和血小板衍化生长因子-BB(PDGF-BB)水平与尿白蛋白排泄率间的关系,探讨TGF-1和PDGF-BB在糖尿病肾病(DN)的发生开展中的作用。方法研究对象共120例。其中健康对照组(NC, UAER<20g/min) 60例,2型糖尿病组90例,将其分为正常白蛋白尿组(NA, UAER<20g/min)、微量白蛋白尿组(MA, UAER 20200g/min)、大量白蛋白尿组(ODN, UAER>200g/min)各30例,测定血压(SBP/DBP)、体重指数(BMI)、空腹血糖(FPG)、空腹C肽(FCP)、糖化血红蛋白(HbA1c)、尿白蛋白排泄率(UAER)及TGF-1、PDGF-BB水平。结果NC、NA、MA、ODN组血清TGF-1水平逐渐增高,T2DM各组血清TGF-1水平均显著高于NC组(Sig=0.0000.037,P<0.05),ODN组显著高于NA组和MA组(Sig=0.000,P<0.05),NA组和MA组比拟差异无显著性(Sig=0.468,P>0.05)。T2DM各组的血清PDGF-BB水平均显著高于NC组(Sig=0.000,P<0.05), MA组显著高于NC组和NA组(Sig=0.000,P<0.05),但ODN组与NA组和MA组比拟差异无显著性(Sig=0.1060.258,P>0.05)。所有对象血清TGF-1和PDGF-BB水平与病程、BMI、SBP、DBP、FBG、FCP、HbA1c、BUN、Cr和UAER均呈正相关(r=0.0750.798,P<0.01);与FCP呈负相关(r=-0.602-0.818, P<0.01); TGF-1和PDGF-BB水平呈正相关(r=0.470,P<0.01)。结论TGF-1介导了DN的病变发生,PDGF-BB在DN的早期致病作用尤为显著,二者在DN开展的不同阶段协同作用,加剧DN进展。【英文摘要】To investigate the relationship between the levels of Transforming growth factor-betal(TGF-1) and Platelet derived growth factor-BB(PDGF-BB) in serum and urinary albumin excretion rate in type 2 diabetes mellitus (T2DM), explore the role of TGF-1 and PDGF-BB in the development of diabetic nephropathy.Method The subjects were 120 cases. One healthy control group (NC, UAER <20g/min) 60 cases,90 cases of type 2 diabetic patients were divided into normal albuminuria group (NA, UAER<20g/min), microalbuminuria group (MA, UAER 20200g/min), macroalbuminuria group (ODN, UAER> 200g/min) of the 30 cases, measured blood pressure (SBP/DBP), body mass index (BMI), fasting blood glucose (FPG), Fasting C peptide (FCP), glycosylated hemoglobin (HbA1c), albumin excretion rate (UAER), TGF-1, PDGF-BB levels.Results NC, NA, MA, ODN TGF-1 levels in serum gradually increased, type 2 DM patients in each group of serum TGF-1 were significantly higher than that of NC group (Sig=0.0000.037, P<0.05), ODN was significantly higher than NA group and MA group (Sig=0.000, P<0.05), NA group and MA group showed no significant (Sig= 0.468, P>0.05). Type 2 DM patients in each group of serum PDGF-BB were significantly higher than that of NC group (Sig=0.000, P<0.05), MA was significantly higher than that of NC group and the NA group (Sig=0.000, P<0.05), but the ODN Group and the NA group and MA group showed no significant (Sig=0.1060.258, P>0.05).
