35、部位、问题及整改意见，限期纠正缺陷。对较严重的质量问题或已形成隐患的问题，应填写“监理工程师通知单”要求承建方及时整改。承建方完成缺陷修复后填写“指令/通知回复单”，报请监理机构复验签认。如承建方不及时改正，情节较严重的，项目总监可以在报请业主同意后，发出“监理工程师指令单”指令部分工程或全部工程暂停施工。如所发现工程质量问题已构成工程事故时，应按规定办理。、分部、分项、各检验批工程完工后，经自检合格，填报“工序质量报验单”，由监理工程师检查签认。、如检查结果不合格，或自检资料的内容与实际不符，监理工程师有权认定等级为“不合格”，责令承建方整改后再次报验。、特殊设计的、或与原设计图变更较大的隐

36、蔽工程，在通知承建方的同时，还应通知设计单位有关人员参加，与监理工程师共同检查签证。、隐蔽工程检查合格后，经长期停工，在复工前应重新组织检查签证，以防意外。13、施工进度监督及报告制度、监督承建方严格按照合同规定的进度计划组织实施，监理机构每月以月报的形式向业主报告各项工程实际进度，并与计划进度相对比。、审查承建方编制的实施性施工组织设计，要突出重点，并使各单位、各工序进度密切衔接。14、投资监督制度、监理机构应督促承建方报送与承包合同相适应的分段概算台帐资料，并随时补充设计变更资料。经常掌握投资变动情况，按期统计分析。、对重大的设计变更或因采用新材料、新技术而增减较大投资的工程，监理机构应及

37、时掌握并报业主，以便控制投资。附件1 组织协调与工地会议制度一、组织协调的主要任务是：项目各方及人际间的关系协调、项目各方分工与配合的协调和项目实施中人、财、物、技术、管理等方面的协调。二、工地会议是施工阶段组织协调工作的一种重要形式。三、工地会议由总监或受其委托的副总监主持。会议所定事项需整理成纪要或备忘录，经总监或或副总监与承包代表签字，对双方均有约束力。四、第一次工地会议参加人员有：业主与承包人的授权代表，各方将在工程项目中担任主要职务的负责人及高级人员。五、第一次工地会议的任务是：1、介绍各方人员及组织机构、各方通报自己的单位正式名称、地址、通讯方式；、业主或业主代表介绍业主的办事机构

38、、职责、主要人员名单，并就有关办事项做出说明；、总监宣布其授权代表的职权，并将授权的有关文件送交业主与承包人。宣布监理机构、主要人员及职责范围，并提交业主和承包人。、承包人书面提出现场（工地）代表授权书、主要人员名单、职能机构框图、职责范围及有关人员的资料。2、检查承包人的开工准备、主要人员是否进场，并提交进场人员名单；、用于工程的材料、机械、仪器和其它设施是否已经进场或何时进场或何时进场，并提交清单；、施工场地临时设施进展情况；、施工测量的基础资料是否复核；、履约保证金及各种保险是否已经办理，并提交已办手续的副本；、为监理工程提供的各种设施是否具备，并提出清单；、检查其它与开工条件有关的内容

40、交底制度一、施工图图纸会审和图纸交底会议有利于参建各方充分熟悉图纸，努力把问题消灭在图纸上。二、会议的召集1、施工图会审和技术交底由业主或由业主委托监理单位召集和主持，业主、监理、设计、施工等部门参加。2、召集单位需预先通知与会单位，做好充分准备，并将最后确定的会议时间、地点、与会单位及人员正式通知有关方面，应要求承建方提交有关图纸问题的书面意见。三、施工图会审的主要内容：1、施工图设计是否符合经审批的初步设计文件和审批意见；2、施工图设计是否符合有关设计规范；3、施工图设计文件是否齐全、准确；4、对施工图文件中的有关问题的质疑；5、必要与可能的修改意见；6、需要会审的其它内容。四、设计单位进

41、行技术交底的主要内容：1、设计单位进行技术交底的主要内容：2、设计指导思想、主要内容及技术特点；3、设计中采用的新技术；4、对工程施工的技术要求及遵循的有关规范；5、有必要说明的其它问题。五、施工图会审和技术交底会提出的各类与工程有关的问题都必须落实单位和人员及时或限期解决。六、会议纪要由主持单位编写，征求与单位意见并签后印发。涉及设计修改和变更的部分由设计单位出具设计修改（变更）。七、施工图会审和技术交底会议要和设计修改单（变更）一般在三天内需发至有关单位，并作为技术文件的一部分进入工程资料档案。附件3 开工报告审批制度一、承建方做好工程施工准备工作后需填报工程开工报审表送监理工程师审查，经

42、确认并由总监批复开工报审表后，方可开工。二、开工前的准备工作包括：1、施工组织设计（或技术方案）2、劳动力按计划进场；3、机械设备进场；4、管理人员全部到位。5、办妥开工前的各项手续。三、监理单位接到工程开工报审表后，现场总监应及时组织人力检查是否具备开工条件，无论是否具备签署开工报审表的条件，总监在规定的时限内都应该签署意见。四、本制度所指“工程”为单位工程或单项工程。附件4 材料、构件、设备检验及复检制度一、施工所需原材料、半成品、构配件必须持有质保书、产品合格证和有关复试报告方可进场。对已进场的不符合上述要求的原材料、半成品和配件，监理工程师有权责令承建方限期清出施工现场。如果逾期不办，

43、监理有权与业主方协商后直接安排人力将不符合要求的物品清离出现场，所发生的费用由承建方承担。二、监理工程师应按国家现行有关规定、标准对进场的原材料、半成品和构配件进行见证送样或根据合同平行抽检。三、复检程序及要求1、实行见证制度，受检材料由监理人员在现场随机抽样取得。抽取试样，需有承建方人员参加。2、由监理人员和承建方人员一起将受检材料送至具备相应资质的材料检验单位（不具备相应资质材料检验单位的检验结果无效）。3、承建方取得材料检验结果后应尽快报监理人员。四、没有质保书、产品合格证和有关复试制报告或抽检不合格的材料、半成品、构配件，不允许在工程中使用。五、凡采用新材料、新型产品，监理工程师必须检

44、查其技术鉴定文件。附件5 工程变更制度一、项目总监在以下情况下，以“监理工程师通知单（工程变更类）”书面通知承建方执行工程变更：1、业主认为需要而提出工程变更，并委托设计单位提出有效变更文件后，以“业主指令单”转发至本项目监理机构；2、设计单位提出设计变更，经业主审批后，以“业主指令单”转发至本项目监理机构；3、承建方提出设计变更，经项目总监审查，报业主和设计单位协商审批后，以“业主指令单”转发至本项目监理机构。二、承建方接到“监理工程师指令单”后，应按变更要求实施；三、在合同规定的天数内，承建方应向项目总监提交变更工程的“计量申报表”，逾期按合同文件处理。变更费用的计算按合同协议规定的办法执

45、行；四、项目总监收到“计量申报表”后，对变更项目进行计量，确认该项目的最终数量、单价和总价。五、工程变更完成并经监理验收合格后，应根据业主批准的变更费用、按正常的支付程序办理费用的支付手续。六、监理有权制止不适当的工程变更（如变更内容违背设计规范、施工规程等）；七、分包工程的变更应通过总包单位办理。八、工程洽商参照工程变更的程序处理。附件6 隐蔽工程验收制度一、隐蔽工程必须经监理工程师检查、签署验收。二、监理工程师应依据工程施工图纸，列出该工程中隐蔽部分的清单，做到心中有数。三、隐蔽工程施工完毕，由承建方组织进行自检，合格后，填报隐蔽工程验收单交监理人员，监理人员应在规定的时限内进行验收。四、

46、监理人员需对隐蔽工程的施工质量、平面位置、几何尺寸、标高、接口、用材等逐一进行验收，经监理工程师签证后方可封闭、掩盖或进行下道工序。五、隐蔽工程若与设计要求有出入，承建方必须在隐蔽工程验收记录中加以说明和附简图，并要征得设计单位签证认可。否则，承建方应提出整改措施，经设计单位签认后，进行整改合格后方可验收。六、未经查验，即自行封闭、掩盖或进行下一道工序者，监理人员有权责令停工、进行整改。附件7 工程质量事故处理制度一、凡在建设过程中，由于设计或施工原因，造成工程质量不符合规范或设计要求，或者超出验评标准规定的偏差范围，需做返工处理的统称工程质量事故。二、工程质量事故发生后，承建方应及时上报“工

47、程质量问题（事故）报告单”。1、对不影响工程整体结构安全性能的，处理方案简易，返工量小的一般工程质量事故，由承建方研究处理。2、对大质量事故，由监理机构组织有关单位研究处理。3、对重大质量事故，由监理机构组织有关单位研究处理方案，报业主批准后，由承建方进行事故处理。待事故处理后，经监理机构复查，确认无误，方可继续施工。三、凡对工程质量事故隐瞒不报，或拖延处理，或处理不当，或处理结果未经监理机构复查认可的，对事故部分及受事故影响的部分工程视为不合格，不予验工计价，待合格后再补办验收计价。附件8 工程竣工验收制度一、竣工验收的依据是批准的设计文件（包括设计变更），设计规范、施工规范、工程质量验收标准、合同以及协议文件等。二、承建方按照规定编写和提出验收交接文件是申请竣工验收的必要文件，竣工验收文件不齐全、不正确清晰，不能验收交接。三、承建方应在验收前将编好的全部竣工文件及绘制的竣工图，提供监理机构一份，审查确认完整后，报业主，其余分发有关接管、使用单位保管。附件9 内部工作制度
