35、制定的预防措施); 5、危险性较大的分部分项工程及施工现场易发生重大事故的部位、环节的预防监控措施和应急预案(根据本企业业务特点，详细列出危险性较大分部分项工程和事故易发部位、环节及有针对性和可操作性的控制措施和应急预案); 6、生产安全事故应急救援预案(应本着事故发生后及时有效救援原则，列出救援组织人员名单、救援器材、设备清单和救援 （一）变更企业名称应提交以下材料： 1、企业注册地设区市建设行政主管部门关于同意企业更名的公函； 2、企业名称变更申请报告； 3、已变更的企业法人营业执照复原件印件； 4、企业修正后的章程复印件； 5、对原企业债权债务的承诺书； 6、已变更的企业资质证书正、副本
