2、ment for any house use of transportationPROCEDURE1. All transportation arrangement should obtain prior approval from their department head by filling out the Transportation Request Form (example attached) with signature of Department Head.2. The request form should be submitted to GRO as early as po

3、ssible for arrangement.3. GRO and Concierge are responsible for approval of dispatching.4. GRO should collect the Transportation Request Form and put it into transportation Master Schedule for Re-confirmation.(see attached)5. If there are no hotel transportation available due to schedule conflicts,

4、and outside transportation other than bus and trolley bus will be necessary and the Transportation Request Form return back to aspects of department for their own record.6. Outside transportation charges (excepts buses, trolley bus) can only be reimbursed against proper receipts with the approved Tr

5、ansportation Request Form.CONTROL1. All house-use Transportation Request Forms should be recorded summarized by the respective department expenditure on the report and scrutinized by the Financial Controller on monthly basis, irregularities must be reported to the General Manager. 2. The estimate mo

6、nthly used kilometers and gasoline cost of each department should be reported to the Accounts Department for the costing breakdown at the end of each month.3.
