11、l全科医生 响石潭传染性非典型性肺炎应急预案非典型肺炎属于中医“热病”范畴，病因为感受疫毒时邪，病位在肺。其基本病机特点为：热毒痰瘀，壅阻肺络，热盛邪实，湿邪内蕴，耗气伤阴，甚则出现气急喘脱的危象。在此基础上，根据所针对的不同人群以及地区气候特点，分别对易感人群的预防、不同类型和不同病期患者的治疗提供了指导性意见。一、目标 通过本方案的实施，完善组织协调机制和监测网络报告，提高应急处理和医疗救治能力。加强部门间协调，及时沟通信息，做到“早发现、早报告、早隔离、早治疗”，及时发现SARS疫情，控制SARS疫情蔓延，防止流行性感冒（简称流感）大流行的发生，保护广大人民群众身体健康。 预防为主，防治

12、结合；协调配合，分级负责；依法管理，科学防治的原则。二、监测与报告1.按照市卫生局转发的传染性非典型肺炎诊疗方案的要求，发现疑似、临床诊断的SARS首例病例，按照传染性非典型肺炎疫情监测报告实施方案文件中规定的报告内容、方式、时限和程序进行报告。2.发现流感首例病例时应在24小时内，通过传染病疫情监测信息系统进行报告；发生流感暴发疫情时应在1小时内，通过传染病疫情监测信息系统进行报告。3.指定防保科专人负责传染病疫情报告工作（上报区卫生局、区疾控中心）。三、预防和控制1、开展健康教育，提高群众的防病意识。采用多种形式，利用电子滚动屏幕、宣传橱窗等开展向社会公众的健康宣传教育活动，提高群众对SA

14、贯众十克、银花十克、连翘十克、大青叶十克、苏叶十克、葛根十克、藿香十克、苍术十克、太子参十五克、佩兰十克，水煎服，一日两次，连续服用七至十天。四、医疗救治1、病人的诊治（1）配备有一定经验的，经过传染病知识培训的医师，对发热病人进行甄别和鉴别诊断。（2）SARS和流感疑似或确诊病例的转送由医务科统一负责。2、加强医院感染控制根据卫生部下发的医院感染管理规范(试行)、传染病非典型肺炎医院感染控制指导原则(试行)等文件要求，做好医院的感染控制工作；根据医疗废物管理条例和市卫生局印发的关于转发市人民政府批转市环保局关于集中处置医疗废物意见的通知要求做好医疗废物的处理。3、中医治疗：早期：早期患者以热

15、毒袭肺、湿遏热阻为病机特征。临床上分为热毒袭肺、湿热阻遏、表寒里热夹湿三种证候类型。属热毒袭肺证者，宜清热宣肺，疏表通络，可选用银翘散合麻杏石甘汤加减；属湿热阻遏证者，宜宣化湿热，透邪外达，可选用三仁汤合升降散加减，如湿重热轻，亦可选用霍朴夏苓汤；属表寒里热夹湿证者，宜解表清里，宣肺化湿，可选用麻杏石甘汤合升降散加减。中期：中期患者以疫毒侵肺，表里热炽，湿热蕴毒，邪阻少阳，疫毒炽盛，充斥表里为病机特征。临床上分为疫毒侵肺、表里热炽，湿热蕴毒，湿热郁阻少阳，热毒炽盛四种证候类型。属疫毒侵肺、表里热炽证者，宜清热解毒、泻肺降逆，可选用清肺解毒汤；属湿热蕴毒证者，宜化湿辟秽、清热解毒，可选用甘露消毒

16、丹加减；属湿热郁阻少阳证者，宜清泄少阳、分清湿热，可选用蒿芩清胆汤加减；属热毒炽盛证者，宜清热凉血、泻火解毒，可选用清瘟败毒加减。极期：极期患者以热毒壅盛，邪盛正虚，气阴两伤，内闭外脱为病机特征。临床上分为痰湿瘀毒、壅阻肺络，湿热壅肺、气阴两伤，邪盛正虚、内闭喘脱三种证候类型。属痰湿瘀毒、壅阻肺络证者，宜益气解毒、化痰利湿、凉血通络，可选用活血泻肺汤；属湿热壅肺、气阴两伤证者，宜清热利湿、补气养阴，可选用益肺化浊汤；属邪盛正虚、内闭喘脱证者，宜益气固脱、通闭开窍，可选用参附汤加减。恢复期：恢复期患者以气阴两伤，肺脾两虚，湿热瘀毒未尽为病机特征。临床上分为气阴两伤、余邪未尽，肺脾两虚两种证候类型

17、。属气阴两伤、余邪未尽证者，宜益气养阴、化湿通络，可选用李氏清暑益气汤加减；属肺脾两虚证者，宜益气健脾，可选用参苓白术散合葛根芩连汤加减。五、预警和应急发布预警的判定、应急预案的启动和应急响应：按照北辰区防治传染性非典型肺炎工作应急预案(北辰政发200351号)执行。六、保障措施与应急准备1、分析疫情趋势，判定工作方案认真总结SARS防治工作经验，结合实际情况，科学分析疫情趋势，有针对性地提出应对之策。2、组织保障中医院成立SARS和流感防治领导小组，负责全院SARS和流感防治工作的组织协调、监督检查、信息管理工作。成立专家救治组和救治队伍。3、做好物资储备做到防治人员、治疗药品、防护设备、消
