12、不明原因肺炎应急处置方案第一章 总则第一条 为科学、规范、有序地开展不明原因肺炎防治工作,落实“早发现、早报告、早隔离、早治疗”的防治原则,确保疫情一旦发生,及时采取有效措施迅速控制疫情,保障广大人民群众的健康和生命安全,维护社会稳定,促进经济发展,根据中华人民共和国传染病防治法、突发公共卫生事件应急条例、传染性非典型肺炎防治管理办法和全国突发公共卫生事件应急预案等法律法规,制定本预案。第二章 应急组织机构与职责任务第二条 组织机构组建防控不明原因肺炎领导小组、疫情处置队和应急处置业务技术组。(一)不明原因肺炎应急领导小组组 长:宋险峰副组长:王 盾 李连玉 陈雪侠 高长喜 王恒娟 李长江 崔

14、田健 6、疫情报告、信息组:组长:杨春华组员:李 朵 于跃红 包春瑜第三条 职责任务(一)防控不明原因肺炎应急领导小组:在洮北区卫生局的统一领导指挥下,负责组织、协调并落实各疫情工作组的应急处置工作,督促、指导各工作组开展应急处置工作。(二)疫情处置队各工作组1、医疗救治组:负责对来院就诊患者进行初步诊断与医疗救治,并向医务科及分管院长进行病情汇报。2、流调检诊组:负责对报告的疑似不明原因肺炎病例进行流行病学调查与线索追踪及后续调查工作;综合评估分析,初步判断、提出初步预防控制策略措施意见和是否启动本预案的建议,经应急领导小组审查同意后向洮北区卫生局、洮北区疾病预防控制中心报告。3、现场消毒处
15、理组:负责承担和指导相关科室和个人对一切相关人员、场所、物品、环境及传播媒介实施隔离、控制和消毒等预防控制措施。3、采样检验组:根据流调检诊组对疫情性质、波及范围及相关因素的初步判断,负责采集标本。(门诊、住院不明原因肺炎疑似病例(确诊和疑似病例)的标本由收治医院负责采集,须在入院后2448小时内完成);密切接触者、社区健康人群和不明原因肺炎出院病人的标本由站采样检验组采集。标本采集运送严格按不明原因肺炎病人标本的采集、运输和保藏指导原则执行。4、后勤保障组:负责不明原因肺炎疫情应急处理所需的车辆安排,药品、器械、防护器材、预防药品、消毒药械等的计划、购置、供应和用后处理工作;根据阶段性工作的
16、需要,随时补充以保证必要的储备。5、疫情报告、信息组:负责不明原因肺炎疫情各种信息资料收集、汇总、上报及反馈;随时保证疫情网络传输正常。制备健康教育宣传资料。(三)发生疫情时,各组组长安排人员迅速到位,在应急队长统一指挥下,各组按照职责任务负责相应疫情调查处理控制工作。第三章 应急准备第四条 各科室要按照职能分工,制定周密计划并认真实施,按照中华人民共和国传染病防治法、突发公共卫生事件应急条例、传染性非典型肺炎防治管理办法切实做好不明原因肺炎防治应急处置准备工作。经常开展应急处置相关法律法规、专业理论知识、工作程序、技术规范、操作技能的培训,现场与岗位演练;不断提高全体专业技术人员的应急处置素
17、质与能力。第五条 各科室要根据专业和职能分工,保证应急处置所需各种调查表格资料、防护器材、采样检测器材、诊断检测试剂、预防用药品和消毒药品及器械等的充分准备和必要的储备;加强车辆维护保养,随时处于完好状态,保证应急使用。第六条 科室负责人和中级技术职称以上人员及驾驶员随时保持通讯工具的畅通;全体干部职工要保持高度警惕,服从统一指挥与安排,随叫随到;因公因私外出必须请示。第四章 病例诊断及疫情报告第七条 执行职务的医务人员根据传染性非典型肺炎病例和疑似病例临床诊断标准进行诊断。执行职务的医务人员发现传染性非典型肺炎病例、疑似病例按照传染性非典型肺炎疫情监测系统规定的报告方式、时限、程序向医务科及
18、感控科报告。初步报告:感控科收到疫情报告后,应做好记录,主要项目包括疫情发生时间、地点、主要症状、发病人数等,通知流调检诊组迅速开展流行病学调查。经调查核实后,向应急领导小组报告。并按照传染性非典型肺炎疫情监测系统的要求录入有关内容并立即报告洮北区卫生局和洮北区疾病控制中心;同时采取相应的消毒处置控制措施。进程报告:根据现场调查和采取措施情况,及时做出进程报告。结案报告:疫情处理完毕后,做出结案报告。第五章 应急处理程序与要求第八条 实行24小时值班制,值班人员负责病人的接收与疫情报告工作。总值班人员或执行职务的感控科工作人员接到疫情报告,要详细询问疫情发生的情况,处理情况以及报告时间,报告人
19、,联系电话等,填写专用记录表,并立即向相关应急组和分管领导报告。第九条 相关应急组和分管领导确认必须启动应急程序的,应立即出动开展调查处理工作,同时向应急领导小组组长报告。第十条 接到不明原因肺炎疑似病例报告后,流调检诊组应立即按卫生部下发的传染性非典型肺炎流行病学调查指导原则进行流行病学调查。同时按照传染性非典型肺炎密切接触者判定标准和处理原则判定、追踪调查密切接触者和医学观察。综合分析流行病学调查初步结果,属疑似病例诊断标准的,采样检验组按规定采样送检。消毒处置组对疫点监督指导进行严格消毒。第十一条 落实各项预防控制措施在初步调查结果的基础上,同时采取下列预防控制措施:(一)开展健康教育,
20、提高群众的防病意识和自我防护能力。积极做好健康教育工作的各项技术支持,大力开展呼吸道传染病预防知识的宣传,教育群众养成良好的卫生习惯,提高抵御疾病的能力。(二)加强医务人员的防控工作:加强医务人员防护、防止医院交叉感染发生。建立和完善发热病人就诊工作流程。采取以严格的传染源隔离、病区消毒、医务人员个人防护等为主导的预防院内交叉感染的综合性措施,杜绝传染性非典型肺炎医源性感染。(三)对疑似病人立即进行隔离治疗。对密切接触者进行医学观察2周,一般采取居家医学观察,必要时集中观察。(四)疫点消毒处理消毒处置组要及时对行进通道进行消毒处理,消毒工作参照传染性非典型肺炎防治消毒技术执行。(五)病人的诊治
21、已建立的发热门诊和留观室要严格按规范运转,对具有高热、咳嗽、呼吸困难等可疑不明原因肺炎症状的发热病人安排专室接诊。发热门诊配备有一定临床经验的、经过传染性非典型肺炎和传染病知识培训的高年资医师,对发热病人进行甄别和鉴别诊断。真正做到早隔离、早治疗,防范医务人员感染。(六)病人转运与转诊在不明原因肺炎疑似病人转运过程中,司机和医务人员要穿戴隔离衣、手套和符合医用标准的口罩。(七)技术培训各科室要做好业务人员的应急培训工作。主要培训内容为传染病防治法及其实施办法、突发公共卫生事件应急条例及突发公共卫生事件应急预案、传染性非典型肺炎防治管理办法以及其它非典防治规范性文件、非典防治基本知识、诊断标准和
22、治疗原则、疫情报告的程序和基本要求、流行病学调查方法、隔离病区设置及操作规范、隔离防护措施和规范、实验室样品采集与保管规范等。第十二条 所有参加不明原因肺炎应急处理工作的人员必须严守工作纪律和技术规程;认真落实个人防护和消毒隔离措施;服从指挥调配。第十三条 不明原因肺炎疫情妥善处理完毕后,向洮北区卫生局和洮北区疾病预防控制中心报告处理结果;并根据情况向有关部门通报。相关疫情处置工作组写出业务总结和处理报告,对处理过程与结果进行评估,总结经验与教训,提出改进意见和建议。第十四条 感控科在不明原因肺炎疫情应急处理工作结束后,应将下列资料及时整理归档,按规定妥善保存。(一)报告记录;(二)应急处理组
