1、中小企业福利管理制度2012-2-22 目录第一章总则2 第二章保险福利2 第三章各类福利补贴4 第四章假期管理6 第五章其它福利12 第六章罚则13 第七章记录与存档13 第八章附则14 第一章 总则第一条 为规范员工福利管理、安定员工生活、提高工作效率、改善员工福利,提高员工满意度,特制订本制度。第二条 本制度适用于公司集团全体员工。第三条本制度主要包括“保险福利”、“各类福利补贴”、“假期管理”、“其它福利”等内容。第二章 保险福利第四条 社会保险:与公司签订劳动(劳务)合同的员工、依法享有基本养老保险、医疗保险、工伤保险、失业保险、生育保险等社会保险;均按各公司所在地社保政策执行、具体
2、规定如下:(一)新员工在三个月试用期满转正后,第四个月必须缴纳社会保险,员工转正后按要求收齐员工相关资料(3 张正反复印的身份证复印件及1张一寸红底照片)在转正当月15 号之前交人力资源部;(二)社保缴费基数为员工各职等的基本工资,基本工资低于公司所在地的社保最低缴费标准时、以当地最低缴费标准为准;(三)社保基数按国家规定每年进行调整;(四)社保缴费金额为:缴费基数缴费比例;(五)社会保险的新增和减员均以当月办理次月生效,社保个人承担费用于生效当月工资中扣除。(六)无需参保人员:文档编码:CQ3V4T8S4G4 HB9G1L10F9Y6 ZQ6V4E1Z4S2文档编码:CQ3V4T8S4G4

10、时间和工作场所内,因履行工作职责受到暴力等意外伤害的;4、因工外出期间,由于工作原因受到伤害或者发生事故下落不明的;5、在上下班途中,受到机动车事故伤害的;6、法律、行政法规规定应当认定为工伤的其他情形。(八)员工如发生事故伤害,该员工部门人员立即送至最近的医保定点医院就医,且于 24 小时内将事件报告说明附件1(说明上需有三方签字,即至少有 3 个当事人做签名证明)交至人力资源部,并于就医结束2日内将就医病历、缴费单据及工伤者的身份证复印件交至人力资源部;(九)工伤工资核算按基本工资30工伤天数;工伤天数以医院开具的建议休假天数为准;(十)失业金领取资格;1、按照规定参加失业保险,所在单位和

18、可享受商业性福利保险;(二)参加保险的员工,其保费由公司和个人各承担50%;(三)参保员工发生意外伤害事故门诊看病和住院可按规定报销医疗费;(四)凡因发生意外事故而住院或门诊的员工、必须在事发24 小时内、向人力资源部报备,以便向保险公司索赔;(五)申请理赔报销医药费时必须提供与收据相对应的病历、医疗收费发票、医疗诊断证明书、休假证明、当事人身份证等原件和复印件、资料不齐全者不予赔付;(六)因意外伤害住院必须到保险公司指定医疗机构就诊、因意外伤害在门诊看病者除第一天可在就近医疗机构就诊外、从第二天起的后续治疗必须在保险公司指定医疗机构就诊(外地员工就诊必须在当地县级或县级以上公立医院就诊)。第
19、三章 各类福利补贴第六条年资补贴:是根据员工在公司服务年限、公司给予相应年资补贴;(一)工作满1 年,在第 13 个月发放50 元年资补贴,依此类推逐年递增,200 元为上限;(二)总监级以上员工不享受年资补贴;(三)如遇病、事、产假连续超过30 天以上者,按月份递减工龄;例:XX 员工入职日起至今共33 个月,其中 5个月因怀孕休事假,因此该员工工龄为 28 个月。第七条生活补贴:是根据不同岗位给予员工的特殊福利;(一)员工自入职之日起即可享受此福利;(二)生活补贴标准为每餐5 元、补贴金额为:实际出勤天数5 元餐数;文档编码:CQ3V4T8S4G4 HB9G1L10F9Y6 ZQ6V4E1
