1、2023介入超声技术在外科急腹症诊治中的应用进展(全文)摘要急腹症是外科临床常见的急危重症,及时地诊断及正确地处理影响到患者 的预后。传统的急腹症诊断多依赖于患者临床及影像学表现,部分患者甚 至需行剖腹探查以明确病因并予针对性治疗。近年来随着介入超声的发展, 超声造影、超声引导下活检、引流等技术逐步应用于外科急腹症的诊治, 特别是包括有外科医师参与的介入超声技术,切实提高了诊断效率,拓宽 了治疗思路。本文就介入超声技术在外科急腹症诊治中的应用进展进行总 结与评价。急腹症指因腹腔内组织或脏器急性病理变化产生的腹部症状和体征,同时 伴有全身反应的临床综合征,是急诊最常见并且需要紧急诊治的疾病,其
2、中部分患者需要急诊外科干预1。近年来,超声、CT等影像技术的发展 提高了急腹症的术前确诊率,降低了阴性探查率。介入超声技术作为超声 领域的重要分支,将超声与微创干预融合一体,同时由于便携可及、实时 引导的特点,被推荐用于急腹症的诊治。近年来,有学者提倡由外科医师 进行超声检查,可以提高急腹症患者的诊断准确率2。我们通过临床实践 也体会到,基于外科医师在介入超声操作中对局部解剖、病理生理、麻醉 及并发症处理等方面的专业优势,使我们思考介入超声技术是否应成为外 科医师在诊治急腹症中的必备基本功,以提高急腹症的诊治水平。本文述approach to acute abdomen in emergenc
