1、船舶演习及记录范本SHIPS DRILL AND RECORD SAMPLE FORMAT为了规范船舶演习及记录，现将船舶演习细节规范如下，请各轮船长、船员参照执行：For standardization ships drill and record,now we detail its criterion as follow,please refer to and adhere to by each master and chief engineer 一、消防演习 fire fighting drill1、事故发生时的实际应变反应：actual response upon happening(

2、1)当船舶发生火灾时，应立即发出相应部位的消防警报信号，全体船员听到警报信号后，应立即按应 变部署表的分工进行灭火。Sound respective fire alarm immediately when its on fire, the crewmember should take action to fight fire upon receiveing the alarm in accordance with the assigned task indicated in muster list(2)命令现场指挥派出探火人员(或消防员)，迅速查明火源，火灾现况，并报告船长；如有船员，要积 极

3、搜救。Order the on scene commander to arrange fireman for detection of fire immediately to find out the fire sources, fire condition and make known to master； if some crew is involved, to rescue actively.(3)如船舶仍有动力，船长要操纵船舶使着火部位处于下风。Where the ships still powerful, manoeuvre it to maintain the fire site

4、at lee side(4)如船舶航行于沿岸岛屿区域或狭水道，应设法下锚，以避免因施救导致船舶触礁、搁浅。If the vessel navigate along coast or in narrow channel,try to drop anchor so as to avoid vessel from grounding or contacting with rocks etc.(5)如系机舱起火，影响动力或电站供电，应停车，启动应急发电机或使用备用电源。If fire in engine room which affect power or electrical power suppl

5、y, stop engine and start emergency generator or electrical power supply by back up battery(6)如需动用大型C02系统灭火(对机舱/货舱)，If it is need to use C02 system ( to engine room /cargo hold)船长要:the captain should命令三副到大型C02系统灭火站，做好相应准备，并派人监督三副执行，准备完毕向船长报告； Instrut 3rd mate to C02 station to get everything ready,hi

6、s performance to be under supervision by other person, report to master upon ready命令火灾现场人员撤离，部门长清点人数；Order to evacuate al 1 personnel on site and head acount by department leader 命令切断风、油、电，关闭挡火闸门；封闭机舱所有出入口和风门、烟囱百叶窗/封闭货舱和大舱 通风筒。Order to shut off ventilation, oi 1 supply,electrical power,shut down fir

7、e damper； close all engine room access and exits,ventilation doors、 funnel fire flaps/ close down holds ventilation在确认上述步骤都完成后，命令三副施放C02,施放完毕向船长报告。C02施放后，要经过长时间冷 却(1224小时)，才可派人进入封闭舱探火。探火人员应该背带空气呼吸器和耐火安全绳，在有专人 配合下进行探火。When above steps confirmed to be completed, order 3 mate to release C02,and report

8、to master upon release completion, it is needed to cool down for sufficient time after C02 release (12 - 24 hours), then the enclose space can be entered for fire detection. The fireman perform the task must dress the SCBA and safe line, also cooperated by other person specially assigned to the task

9、.(4)因碰撞造成人员伤亡，应进行紧急救治，具体步骤按人员重伤/重病要求处理。If human injury found in the collsion, conduct first aid immediately. The injury to be treated as heavy injury/i1Iness .(5)因碰撞引发火灾，按火灾救助规定执行。Where fire happened due to collision, performed as fire fighting(6)因碰撞发生人员落水，应向落水者抛掷救生圈；有条件应按应变部署表的分工，施放救生艇救人。In case of

10、 man overboard, throw life buoy to the man； if practicalbe, release lifeboat to rescue according to duties assigned in muster list.(7)船长还应指示当值人员做好现场抢险记录(包括对方船名、呼号、船籍港、始发港和目的港、载货情 况、船东名称及地址)，保存好相关海图，以便事故的调查处理。The master should instruct the watchkeeper to make records in relation to the process for de

11、aling with the emergency (including the name of counterpart, cal 1 sign, port of registry and destination, cargo loaded condition, ships owner and address), keep the related charts in order to facilitate the accident investigation)(8)如情况紧急，船长应向就近港口主管机关或海上搜救中心请求救助。If vessel in immediate danger, the m

12、aster should report to the nearest port authority or RCC for assistance(9)发生碰撞后船体受损严重，发生大量进水，船体严重倾斜经抢救无效时，船长可作出弃船决定，具体 操作按弃船规定执行。If ship7 s hull damaged seriously,ingress of water with large quantity and vessel list excessively, master may decide to abandon ship, then follow up action as per abandon

13、ship procedure2、演习时的注意事项：caution note when drilling应注意船舶发生碰撞后的车舵使用，港口及搜救中心中心联络方式，碰撞确认通知书及事故报告、海 事声明等。特别注意碰撞船位不可缺少。Pay attention to the use of rudder and propeller after collision, the communication measure with port and RCC,acknowledge of collision and Accident Report,Sea Protest etc.particularlly t

14、he ships position upon collision.3、演习的标准记录格式：the sample format for drilling record1300 进行船舶碰撞演习，记录碰撞船位 Start collision drill, record collided location1303人员到位，大副点名并检查人员着装及携带设备情况，开启所有船舶照明并检查照明状态正常Crewmember in position, head account by chief mate and checked the condition of crews dressing and materi

15、al taken, switch on deck lighting and checked in good condition 1306进行现场抢险救人演习，船长进行适当的车舵操纵Conducted emergency combat and on scene rescue exercise, the master take use of rudder and propeller appropriately.1315模拟上报碰撞事故，填写碰撞确认通知书及事故报告、海事声明等Simulated to report the collision, completed acknowledgement o

16、f collision and accident report, sea protest etc.1330 演习结束 drill completed九、失控演习 not under command drill1、事故发生时的实际应变反应：actual response upon happening如因风浪大，船舶驾驶困难，发生失控漂航。In case of vessel encountered heavy weather and hard to navigate at sea which give rise to vesselz s not under command and floating

17、(1)应争取使船首迎风浪，船舶作偏顶风浪滞浪航行，等待海况天气的好转，有条件择地抛锚规避。Take measure to against wind with relative bearing about 20-30 degree and keep sluggish to wait for weather condition getting better, if practicable to drop anchor for shelter 检查所有的水密门、舱盖、通风筒、舷窗等，保持水密。Check all watertight door, hatchcover, ventilation duc

18、t, porthole etc to keep watertightness (3)悬挂失控信号。Hoist not under command signal(4)如造成船体损坏和进水严重，具体操作见船体损坏/进水规定处理。If ships hull failure and flooding seriously, the operation refer to structure failure/blooding因主机、舵机或电站故障/损坏，造成船舶失控漂航。Vessel not under comman and floating result from main engine,steering

19、 gear or generator failure/damage(1)应争取使船舶迎风浪，避免横摇。采取的措施可以是：大洋上，用锚机松出锚链34节；沿岸海区，择地抛锚。Take measure to against wind with relative bearing about 20-30 degree to avoidrolling. The measures including： slack anchor chain 3-4 shackle in open sea； drop anchor when coastal navigation.抢修机电设备，尽快恢复动力；具体见机损/故障。P

20、ush to repair electrical equipments, restore power as soon as possible；see engine damage/broken(3)检查所有的水密门、舱盖、通风筒、舷窗等，保持水密。Check all watertight door, hatch cover, ventilation duct,porthole etc to keep weitertightness (4)悬挂失控信号。Hoist not under command signal2、演习时的注意事项：caution note when drilling检查所有水密

21、门、失控信号的状况，注意填写海事事故报告、海事声明的事项Check all watertight doors,condition of not under command signal,pay attention to complete the accident report and sea protest etc.3、演习的标准记录格式：the sample format for drilling record1300进行船舶失控演习，失控船位，悬挂失控信号Start not under command drill,record chips position, hoist signal130

23、ll completed十、人员受伤human injury1、事故发生时的实际应变反应：actual response upon happening(1)发现有人受伤，应立即呼救；抢救人员迅速进入现场，将受伤者抬离现场，骨折者应先进行固定。When human injury found, call for rescue immediately； the rescuer enter into the place and take away the injuried person, for fracture,it must be fixed first .(2)受伤者离开现场后，放置在通风地点，

24、船医或其他船员应积极进行人工呼吸等促使受伤者恢复心跳 和自主呼吸的抢救，抢救应连续半小时以上。如经抢救，受伤者仍无自主呼吸、未恢复心跳、瞳孔 放大、皮肤出现死人斑点等情况时方可判定死亡，停止抢救。The person insuried to be placed at well ventilated place after taken away from the insuried place, shipc doctor or other person should administer the artificial respiration to the injuried person activ

25、ely etc so that the injuried person restore his heartbeat and respiration, the first aid should be lasted at least half hour. If the injuried person sti 11 has no respiration and heartbeat but pupil of his eyes magnified and cadaver spots etc found on his skin,then stop rescue.(3)抢救过程应做好记录，同时向公司应急指挥

26、中心报告抢救情况。Proper record to be made for the whole process and report the situation to company emergency resonse center.2 演习时的注意事项：caution note when drilling注意确认人员受伤部位、性质，以确定救治方法Take note to identify the injuried location,nature so as to ascertain rescue measure3、演习的标准记录格式：the sample format for drillin

27、g record1300 进行人员受伤演习 start human injury drill1303人员到位，大副点名并检查人员着装及携带设备情况Crewmember in position, head account by chief mate and checked the condition of crews dressing and material taken1306将模拟受伤者用担架抬到病房.Simulate to take the injuried person to hospital by stretcher1310对受伤者进行施救，作好记录，填写相关文件Rescue the

28、person injuried and record made properly, complete relevant documents 1330 演习结束 drill compltcd4、防海盗演习 anti piracy drill1、事故发生时的实际应变反应：actual response upon happening(1)航经海盗出没区域，要加强了望，保持船内通信畅通，夜间增派人员巡逻值班。When vessel navigate cross the pirate area, keep sharp lookout and maintain smooth internal commun

29、ication, increase crew for patrol watch at night.(2)封锁要害部位和通道，发现情况及时向沿岸主管机构和附近船舶报警，请求援助，并向公司调度报 告动态。Blank off vital places and passageway,report og coastal state and vessels in the vicinity, compeiny operator if any found.(3)海盗已上船或船上发生暴力伤害事件，船长应以最快速度用SSAS及其他信号发出遭遇海盗袭击/发 生暴力的报警信号(包括燃放求救火箭信号)，请求救援。Upo

30、n the pirates boarding vessel or violence happing, the master should send alarm signals in relation to pirate attacking /violence encountering by SSAS or other measures to ask for assistance(4)船长应将全体人员集中一起，相互保护，相互照应；避免与海盗正面冲突，尽力拖延时间，等待救援。 The master should muster all the crew together, protect and a

31、ttend one another； avoid to conflict with pirates；try to prolong time and wait for assistance.(5)如可能，人为制造机电设备故障(如关闭油柜速闭阀)，迫使海盗劫船阴谋不能得逞。If possible, break down the onboard electical equipment intentionally (such as shut off quick closeing valve in oi1 tank), force the pi ratez s hijacking failure(6)海

32、盗离船后，应将情况(地点、海盗人数、受损情况、有无人员受伤等)报告公司应急指挥、当地港口 主管机关和我驻当地港口的使领馆。After the pireites disembarkation, the master should report to companyz s emergency response commander,local port authority and Chinese consulate regarding the situation includingposit ion,ammount of pirate, damage condition,human injury e

33、tc.2 演习时的注意事项：caution note when drilling应参照保安计划进行，演习时还应对使用器材进行检查Please refer to SSP and check the condition of security equipment3、演习的标准记录格式：the sample format for drilling record1300 进行防海盗演习 Start anti-pirate drill1303人员到位，大副点名并检查人员着装及携带设备情况，并对使用器材状况进行检查，良好 Crewmember in position, head account by ch

34、ief mate and checked the condition of crews dressing and material taken, all in good order1306按照SSP中的要求进行演习Drill according to SSP1330 演习结束 Drill completed十二、货物移动或泄露cargo shifting or leakage1、事故发生时的实际应变反应：actual response upon happening(1)当货物移动/落海时，船长应调整船舶航向和航速，以减少横摇角度，避免谐振。In case of cargo shifting/o

35、verboard to sea, the master should adjust ships course and speed to reduce rolling and avoid resonance(2)条件允许时，应选择安全地点抛锚，并调整油水，尽可能使船舶处于正浮状态。If it is practicable, drop anchor at safe place and adjust onboard oi 1 and water, make great effort to maintain the vessel in positive floating situation.(3)引起

36、倾侧，无法利用油水调整时，应报告公司和就近海上搜救中心，请求救援。If give rise to list, adjusting ships oil and water become impossible, report to company and nearest RCC for assistance(4)发生货物落海，应向公司应急指挥和就近港口主管机关报告同时向附近的船舶通报。When cargo overboard to sea, report company and nearest port authority, inform vessels in the vicinity(1)对舱面

37、的货物泄漏建议使用大量水将泄漏物质冲洗下船。To the cargo leakage on deck, suggest to wash it into sea by copious water(2)发生火灾，建议使用水来灭火，并操纵船舶使着火部位处于下风。In case of fire, suggest to extinguish the fire by water and manoeuvre the ship to make the fire location at lee side(3)舱内火灾，如果是密封货舱，切断通风设备，使用固定式C02等灭火装置。If fire in hold wh

38、ich was sealed, shut off vetilation ,take use of the fixed C02 system (4)抢险人员应穿着防护服和使用空气呼吸器。The emgergency team member should dress protection suit and breathing apparatus (5)万一人员接触危险物质(如皮肤、眼睛)，应立即使用大量的水冲洗受影响部位1015分钟。Once there arc somebody contacted with dangerous substance( such as skin, eyes etc)

43、、事故发生时的实际应变反应：actual response upon happening（1）立即对封闭场所进行大量通风，Vetilate the space as strong as possible immediately（2）施救人员穿戴好消防员装备，带好安全绳对被困者进行施救Rescuers dressed firemans outfit and safety line to start rescue of person was tripped （3）救出的受伤人员，按照人员受伤事故进行处理Take out the injuried person, deal with accordin

44、g to the personal injury procedures2、演习时的注意事项：caution note when drilling对封闭场所进行大量通风十分重要，进入封闭场所应带好安全绳，记好联络信号，进入前，按照检查 表，相关人员填好并签字.It is utmost importance to venti late the space as strong as possible before entry, the safety line must be taken and communication signal must be well familiarized. 47、se upon happening（1）发现有求救信号、落水遇难者/艇筏，值班驾驶员应立即报告船长。If found the distress signal, survival/raft at sea,the 00W on the bridge must report to mcister without dekiy（2）立即改向驶往遇难地点/遇难艇筏，同时发出警报、主机备车，作好施放救助艇的准备。Alter course and proceed to the distress location/raft, meanwhi1e sound alarm, stand by engine,get

48、ready to release the rescue boat(3)开启雷达搜索目标(在9GH：频率工作的雷达)。Switch on radar for searching target( on 9 GH radar)(4)抵达遇难地点附近，按应变部署表规定的分工，施放救助艇(救生艇)救助。Arrval of vicinity of distress location, lower down rescue boat to rescue as per muster list (5)其他船员做好急救准备，进行抢救。Other crew to get ready for first aid(6)如难船尚无沉没危险，须其他救助，如本船有救助能力，应根据国际惯例(公约)签订相应救助合同， 并执行；如本船无救助能力，可在旁监护，并向公司应急指挥及附近海上搜救中心报告，得到指令 后，方可撤离。If vessel in distress is not in danger for sinking and requirment other salvage, conduct salvcige according to internaltional customary salvage agreement signed if own v
